If client answered “YES” to any of the above questions, client should stay at home and not visit the service location. Please advise client to contact a health-care provider and/or call 811.
If client answered “NO” to all the questions above, please advise the client to attend the scheduled appointment 10 minutes early for an entry check-in onsite. Due to the occupancy limit and physical distancing, the client is expected to attend the appointment alone (except clients with special needs). We ask clients to wear a mask when visiting the service location.
Also, if the client develops any symptoms of COVID-19 by the time of the appointment, please advise the client to contact the service location immediately and the staff will reschedule the appointment accordingly.
